Gujarat Birth Certificate Form - Summary
Gujarat birth certificate form is the official birth reporting application used to register a birth with the local municipal corporation, municipality, or gram panchayat in Gujarat. In line with the Registration of Births and Deaths (Amendment) Act, 2023, the digitally signed birth certificate serves as single document proof of date and place of birth for school admission, voter ID enrollment, driving licence, passport issuance, and government jobs.

Documents to Attach with Gujarat Birth Certificate Form
Attach self-attested photocopies of the following records with the filled form:
- Discharge summary or birth slip issued by the hospital, maternity home, or medical practitioner
- Aadhaar cards of both father and mother
- Residential address proof in Gujarat such as electricity bill, property tax bill, or ration card
- Affidavit by parents or verification from the Talati cum Mantri for home births
How to Fill and Submit the Form
STEP 1 – Fill the child’s date of birth, gender, and name in Gujarati or English script.
STEP 2 – Record the father’s and mother’s names, Aadhaar numbers, and permanent address.
STEP 3 – Mention the place of birth along with hospital name or house address.
STEP 4 – Submit the form within 21 days at the municipal ward office or gram panchayat without any fee. You can also apply online through enagar.gujarat.gov.in or digitalgujarat.gov.in.
STEP 5 – For delayed registrations between 21 and 30 days after birth, pay the late fee. Registrations after 30 days up to 1 year require permission from the Mamlatdar or Sub-Divisional Magistrate.
