Rajasthan birth certificate form is the official birth reporting document (Form 1) issued by the Directorate of Economics and Statistics, Government of Rajasthan, for civil registration of births. In accordance with the Registration of Births and Deaths (Amendment) Act, 2023, the digitally signed birth certificate issued through the Pehchan portal serves as single document proof of date and place of birth for school admission, government employment, voter list registration, and passports.

Documents to attach with Rajasthan Birth Certificate Form
Attach self-attested photocopies of these documents with the application:
- Institutional birth certificate or discharge slip from the government or private hospital
- Jan Aadhaar card or Aadhaar card of both father and mother
- Residential address proof in Rajasthan such as ration card or electricity bill
- Affidavit by parents or verification report by the Gram Vikas Adhikari for domestic births
How to Fill and Submit the Form
STEP 1 – Fill the child’s date of birth, sex, and name in clear Hindi or English script.
STEP 2 – Enter the Jan Aadhaar number, father’s name, mother’s name, and permanent residential address.
STEP 3 – Complete the statistical section on page 2 covering parental literacy, occupation, and type of delivery.
STEP 4 – Submit the form within 21 days at the Gram Panchayat office or Nagar Palika / Nagar Nigam registrar office without any fee. You can also apply online through pehchan.raj.nic.in or at the nearest e-Mitra kiosk.
STEP 5 – If submitting between 21 and 30 days after birth, pay the late fee. Registrations after 30 days up to 1 year require permission from the Tehsildar or Sub-Divisional Officer.
