How to Fill and Submit the Form
STEP 1 – Fill the child’s date of birth, gender, and full name in block letters.

STEP 3 – Mention the place of birth along with hospital name or residential address.
STEP 4 – Submit the form within 21 days at the municipal office counter or gram panchayat without any fee. You can also apply online through birthdeath.odisha.gov.in or at the nearest Mo Seva Kendra.
STEP 5 – For registrations between 21 and 30 days after birth, pay the prescribed late fee. Registrations after 30 days up to 1 year require permission from the Sub-Collector or Tehsildar.
