Documents to Attach with Haryana Birth Certificate Form
Attach self-attested photocopies of the following records with the filled form:
- Hospital discharge summary or birth slip issued by the government hospital or private nursing home
- Parivar Pehchan Patra (Family ID) of Haryana
- Aadhaar cards of both father and mother
- Residential address proof such as electricity bill, voter ID, or ration card
- Affidavit by parents or verification report from the Gram Sachiv / Anganwadi worker for domestic births
How to Fill and Submit the Form
STEP 1 – Write the newborn’s date of birth, gender, and full name in capital letters.
STEP 2 – Record the father’s and mother’s names, Aadhaar numbers, and 8-digit Parivar Pehchan Patra ID.
STEP 3 – Mention the place of birth along with hospital name or residential address.
STEP 4 – Submit the form within 21 days at the municipal committee or PHC office without any fee. You can also apply online through saralharyana.gov.in or at the nearest Antyodaya Saral Kendra.
STEP 5 – For registrations between 21 and 30 days after birth, pay the prescribed late fee. Registrations after 30 days up to 1 year require permission from the Sub-Divisional Magistrate or District Registrar.
