Assam birth certificate application form is the official document used to register a newborn and obtain a birth certificate from the Health and Family Welfare Department, Government of Assam. In line with the Registration of Births and Deaths (Amendment) Act, 2023, the issued birth certificate serves as single document proof of date and place of birth for educational admissions, voter card registration, driving licence, passport issuance, and government jobs.

Documents to attach with Assam Birth Certificate Form
Attach self-attested photocopies of the following records with the filled form:
- Discharge summary or birth slip issued by the hospital, primary health centre, or nursing home
- Gaonburah certificate or village head certificate for home births
- Aadhaar cards or voter ID cards of both father and mother
- Residential address proof in Assam such as electricity bill or ration card
How to Fill and Submit the Form
STEP 1 – Fill the child’s date of birth, gender, and name in capital letters.
STEP 2 – Record the father’s and mother’s names, Aadhaar numbers, and permanent address.
STEP 3 – Mention the place of birth along with hospital name or residential address.
STEP 4 – Submit the form within 21 days at the Public Facilitation Centre (PFC) at the Circle Office or apply online through sewasetu.assam.gov.in without any fee.
STEP 5 – For delayed registrations between 21 and 30 days after birth, pay the prescribed late fee. Registrations after 30 days up to 1 year require permission from the Circle Officer or Sub-Divisional Magistrate.
