How to Fill the Voter Form 8

The application should be addressed to the Electoral Registration Officer of the Constituency where the applicant’s name is already listed. Make sure to include the name of the Constituency in the appropriate space.
In Part-I of the application, clearly write your name as it appears in the Electoral Roll. If your initials are abbreviated and you want them fully expanded, you can write your full name. Write your first name (without the surname) in the first box, and your surname in the second box. If you don’t have a surname, just put in your given name. Avoid mentioning your caste unless it is part of your name. Also, do not include titles like Shri, Smt., Kumari, Khan, Begum, or Pandit.
