Those who are availing any other financial assistance under any other Govt. scheme are also eligible for this scheme.
This scheme is meant for kidney patients belonging to BPL families who are undergoing dialysis at least once in a month. The assistance is Rs.1100/- per month.

1. The patient should be from a BPL (Below Poverty Line) family.
2. The applicant must be a nationalized bank account holder.

6. രോഗിയുടെ വാർഷിക വരുമാനം (വില്ലേജ് ആഫീ സിൽ നിന്നും ലഭിക്കുന്ന വരുമാന സർട്ടിഫിക്കറ്റ് ബി.പി.എൽ. റേഷൻ കാർഡിന്റെ കോപ്പി/ ബി.പി.എൽ സർട്ടിഫിക്കറ്റ് ഹാജരാക്കുക)

(ഡി) ഇപ്പോൾ ചികിൽസിക്കുന്ന ഡോക്ടറുടെ പേര്
(ഇ) ഇപ്പോൾ ചികിൽസിക്കുന്ന ആശുപത്രിയുടെ പേര്

11. വൃക്ക കരൾ മാറ്റി വയ്ക്കൽ ശസ്ത്രക്രിയയ്ക്ക ശേഷം ബന്ധപ്പെട്ട ആശുപത്രികൾ നൽകുന്ന ഡിസ്ചാർജ് ഷീറ്റിന്റെ ഗസറ്റഡ് ആഫീസർ സാക്ഷ്യ പ്പെടുത്തിയ പകർപ്പ് ഉളളടക്കം ചെയ്യേണ്ടതാണ്.
