This one-page Gujarati PMJJBY application form is the consent-cum-declaration form issued under the Department of Financial Services’ Jan Suraksha scheme in 2015. It records the account holder’s details and consent for premium auto-debit. This is an older form, not the current enrollment form.
Under current PMJJBY rules, eligible bank and Post Office account holders aged 18 to 50 can join for ₹2 lakh cover at an annual premium of ₹436.
