When to use BPSC self declaration form
BPSC releases this declaration format along with exam day guidelines for competitive recruitment exams, including teacher recruitment exams (TRE) and combined competitive exams (CCE). It is accepted only for identity verification when there is a printing or upload error on the admit card.
Documents to carry with the form
Carry these items along with the completed form to the examination centre:
- Two recent color passport-size photographs, with one pasted on the form and attested by a Gazetted Officer
- Original photo identity proof such as Aadhaar card, PAN card, or voter ID card
- One self-attested photocopy of the photo identity proof
- Printed copy of the BPSC e-Admit Card
How to fill and submit the form
STEP 1 – Print the declaration format on clean A4 size paper.
STEP 2 – Fill in your name, father’s name, registration number, and roll number in clear handwriting.
STEP 3 – Paste your recent passport-size photograph in the designated box on the right side.
STEP 4 – Get the pasted photograph attested with the official signature and stamp of a Gazetted Officer.
STEP 5 – Sign the declaration statement in both Hindi and English in the presence of the room invigilator at the exam centre.
STEP 6 – Submit the attested form and identity photocopy to the Centre Superintendent for entry approval.
