Delhi Birth Certificate Form - Summary
Delhi birth certificate form is the application form used to register a newborn and obtain a birth certificate from the Municipal Corporation of Delhi (MCD), New Delhi Municipal Council (NDMC), or Delhi Cantonment Board. Under the Registration of Births and Deaths (Amendment) Act, 2023, the digitally issued birth certificate serves as single document proof of date and place of birth for school admission, voter ID, driving licence, passport issuance, and government jobs.

Documents to attach with Delhi Birth Certificate Form
Attach self-attested photocopies of the following records with the filled form:
- Discharge summary or birth slip issued by the hospital, maternity home, or medical centre
- Aadhaar cards of both father and mother
- Proof of residence in Delhi such as electricity bill, voter ID, or passport
- Affidavit by parents or hospital verification letter for home deliveries
How to Fill and Submit the Form
STEP 1 – Write the newborn’s date of birth, gender, and full name in capital letters.
STEP 2 – Record the father’s and mother’s names, Aadhaar numbers, and residential address in Delhi.
STEP 3 – Mention the place of birth along with hospital name or residential address.
STEP 4 – Submit the form within 21 days at the zonal MCD office, NDMC counter, or apply online through mcdonline.nic.in or edistrict.delhigovt.nic.in without any fee.
STEP 5 – For registrations between 21 and 30 days after birth, pay the late fee. Registrations after 30 days up to 1 year require permission from the Sub-Divisional Magistrate.
