Voter ID Card Form 6 - Summary
Voter form 6 is an application for inclusion of name in the Electoral Roll for the eligible citizens of India who have attained the age of 18 years can apply for a Voter ID card. Fill and submit Form 6 along with the needed documents.
मतदाता सूची में नाम शामिल करने के लिए फॉर्म 6 भरा जाता है जिस व्यक्ति की उम्र 18 वर्ष या उससे अधिक की है और अपना नाम मतदाता सूची में जोड़ना चाहता है, तो उसे फॉर्म 6 भरकर जमा करना होगा। संबंधित बीएलओ (बूथ लेवल ऑफिसर) को फॉर्म जमा करें। इसके साथ ही इसे चुनाव आयोग द्वारा निर्धारित मतदान केंद्र में भी जमा किया जा सकता है।
Voter Form 6 – Apply Online for New Voter ID Card
STEP 1: Go to the Voters’ Services Portal using the link voters.eci.gov.in
STEP 2: At the homepage click on “Form 6” under New Voter Registration
STEP 3: After clicking on that link you have to click on “Sign Up” button
STEP 4: Now enter the mobile number and captcha code, then click on continue button
STEP 5: Then enter your First name and OTP received on your mobile to verify your profile and your user ID successfully created
STEP 6: Once your ID created, you can login with your mobile number, password and OTP on the portal homepage.
STEP 7: Select Fill Form 6 from the New Registration for General Electors section.
STEP 8: Fill your personal details, family details, address, DOB, Aadhaar and contact info. Upload supporting documents and choose Preview and Submit.
STEP 9: Then you have to fill the declaration form and check everything once again and enter the captcha code to Submit your Form 6 request.

- Government-issued service card
- Public sector bank passbook
- Rental/ Lease Agreement or Property Document with deed and photograph
- Arm’s License
- SC/ ST/ OBC official document
- Railway Identity card
- Student Identity Card
- Document for Physical Handicap
- Freedom fighter identity card
- Pension Document etc.
Proof of Age:
- SSLC Certificate
- Birth Certificate
- Driving License
- any other suitable document that specifies your age.
Two recent passport-sized photographs:
- This photo would appear on the Elector’s Photo Identity Card. The photo should be taken in less than six months before submission.
