Medical Leave Form Certificate - Summary
A Medical Leave Form Certificate is crucial for any employee looking to take medical leave. This certificate serves as a written statement from a physician that verifies the results of a medical examination of a patient, issued by a doctor. Employees are entitled to **one month of medical leave** for every 18 months of service, which is compensated at half their wages. Furthermore, anyone covered under the Act can also take sick leave, which is not less than one-eighteenth of their service period, and this too is compensated at half of the wages.
Eligibility for Medical Leave
To be eligible for medical leave, an employee must have worked for the company for at least 12 months. This period can be extended if the employee is receiving medical care or has been hospitalized. If the medical leave is requested for a family member, the patient must have been in the hospital for a minimum of 3 consecutive days. Additionally, an application for medical leave due to pregnancy should be submitted to the HR department.
Special Cases
In cases involving chronic illnesses, the company is obligated to conduct two master reviews. If an employee experiences complications such as an abortion, the medical records must include details of the patient’s visits and any related births, which should be compensated appropriately by the company.
