Documents to attach with birth certificate form
Submit self-attested copies of the following documents along with the filled form:
- Proof of birth letter or discharge slip issued by the hospital or nursing home
- Aadhaar card or voter ID of both parents
- Address proof of parents such as electricity bill, ration card, or rent agreement
- Marriage certificate of parents, where available
- Affidavit by parent or informer if the birth took place at home
How to fill and submit the form
STEP 1 – Fill the child’s date of birth, sex, and name (if already decided) in block letters.
STEP 2 – Enter the full names and Aadhaar numbers of the father and mother, along with their permanent residential address.
STEP 3 – Record the exact place of birth, whether a medical institution or private residence.
STEP 4 – Sign the declaration section as the parent or authorized informer.
STEP 5 – Submit the form within 21 days of birth at the local registrar office or through the official portal crsorgi.gov.in without any fee. Registrations after 21 days require late fees and approval from the designated magistrate.
