Telangana Birth Certificate Form - Summary
Telangana birth certificate form is the application form used to register a birth and obtain an official birth certificate through MeeSeva centres, the Greater Hyderabad Municipal Corporation (GHMC), or municipalities in Telangana. In accordance with the Registration of Births and Deaths (Amendment) Act, 2023, the issued birth certificate serves as single document proof of date and place of birth for educational admissions, voter card registration, driving licence, passport issuance, and government jobs.

Documents to Attach with Telangana Birth Certificate Form
Attach self-attested photocopies of the following records with the filled form:
- Institutional birth report or discharge slip issued by the hospital, maternity home, or nursing home
- Aadhaar cards of both father and mother
- Residential address proof in Telangana such as electricity bill, property tax receipt, or food security card
- Affidavit by parents or verification certificate from the local corporator or ward member for home births
How to Fill and Submit the Form
STEP 1 – Fill the child’s date of birth, gender, and full name in capital letters.
STEP 2 – Record the father’s and mother’s full names, Aadhaar numbers, and permanent address.
STEP 3 – Mention the place of birth along with hospital name or residential address.
STEP 4 – Submit the form within 21 days at the nearest MeeSeva centre or municipal circle office without any registration fee. You can also apply online through ts.meeseva.telangana.gov.in or ghmc.gov.in.
STEP 5 – For delayed registrations between 21 and 30 days after birth, pay the prescribed late fee. Registrations after 30 days up to 1 year require permission from the Revenue Divisional Officer.
