How to Fill the Voter Form 6
- The application should be addressed to the Electoral Registration Officer of the Constituency where you want to register. Don’t forget to mention the name of your Constituency in the space provided.
- Name (With Documentary Proof): Your name should be provided exactly as it will appear in the Electoral Roll and on the Elector’s Photo Identity Card (EPIC). Write your full name, placing the surname in the second box. If you don’t have a surname, just write your first name. Remember, caste should only be mentioned if it’s part of your name or surname.
- The age of the applicant must be eighteen or more as of 1st January of the year in which the electoral roll is being revised. Make sure to provide your age in years and months.
- Fill in your sex completely, like male or female. If you prefer, you can choose ‘Other’ to describe your gender.
- Provide your date of birth in the specified format (dd/mm/yyyy). You will need to attach proof of your date of birth, such as:
- A Birth Certificate from the Municipal Authorities or District Office of the Registrar of Births & Deaths or a Baptism certificate; or
- A Birth Certificate from the last Government/Recognized school or any other authorized educational institution you attended; or
- If you don’t have any of these documents, you must attach a declaration in the prescribed format from a parent already registered in the Electoral Roll to support your age. This format will be provided on request.
- For unmarried female applicants, the name of your Father or Mother needs to be mentioned. If you are married, include your Husband’s name and strike out the options that do not apply.
- In the space provided, write your complete postal address, including PIN code, where you normally reside and wish to get registered. Attach proof of your ordinary residence, such as:
- A current Pass Book from a Bank/Kisan/Post Office; or
- Your Ration Card/Passport/Driving License/Income Tax Assessment Order; or
- The latest bill for Water/Telephone/Electricity/Gas Connection addressed to you or your immediate relative, such as parents.
