- A passport-sized photograph of the child.
How to Apply Aadhaar Form 5-18
Visit the Enrolment Centre
After locating the nearest enrolment centre, bring the child along with all necessary documents. You’ll need to fill out an enrolment form with personal details of the child, including name, date of birth, and address.
Data Collection
Once you fill out the enrolment form, the child’s photograph will be taken. Note that fingerprints and iris scans are not captured for children under 5 years. After a child turns 5, updating biometrics becomes mandatory.
If a child is below 5 years old, a parent or guardian must authenticate on their behalf and provide consent for enrollment by signing the form, as per UIDAI guidelines.
For NRI children, a valid Indian passport must be shown as Proof of Identity (PoI). For Indian residents, valid proof of relationship documents like a birth certificate, along with the Aadhaar of the parent or guardian, can be used for enrollment.
Acknowledgement Slip
Once the biometric data has been collected, you will receive an acknowledgment slip containing the enrolment ID. This ID helps in tracking the status of the Aadhaar card application.
