Passport Application Form - Summary
The Passport Application Form is a crucial document that consists of two main forms: the Passport Application Form and the Supplementary Form. It’s important to fill out the Supplementary Form only if the references in the Passport Application Form apply to you. If not, you can leave those parts empty.
