i) Application Form:

• The Block Development Officer, if the applicant lives in a rural area.
• The Sub-divisional Officer for Municipal/Notified Areas outside Kolkata, or the Municipal Commissioner if residing within the Kolkata Municipal Corporation.

Verification of Applications:

Processing of Applications:
