This one-page community certificate application, archived in Tamil Nadu Agricultural University’s Agritech forms collection, has Tamil and English labels. It asks for the applicant’s name, parent or husband’s name, sex, address, ration card number, school leaving or transfer certificate, parents’ community or school certificate, application date and signature.
This is a generic, older Community Certificate form, not the current online OBC Certificate application (REV-115). Tamil Nadu lists REV-115 under Revenue Department services on e-Sevai. The archived form page does not say that this paper copy is current or accepted for new applications.

For an OBC certificate, follow the current instructions on the Tamil Nadu e-Sevai portal: https://www.tnesevai.tn.gov.in/citizen/portallogin.aspx. TNeGA’s FAQ says applicants need a Citizen Access Number (CAN) to use e-Sevai services.
