Niramaya Health Insurance Application Form Kerala PDF

This is an application form for health insurance coverage up to Rs. 1 lakh to disabled persons issued by the Social. Justice Department of Kerala and this application form is available to download in PDF format at sjd.kerala.gov.in through online mode or it can be directly download from the link given below in PDF format.

The state cabinet committee has also approved the implementation of Niramaya Health Insurance Scheme to provide health insurance to physically handicapped people. According to guidelines of the department, govt. will provide insurance cover of Rs. 1,00,000 to handicap persons in each district of the state. All the persons with disability belonging to Above Poverty Line (APL) / Below Poverty Line (BPL) category would be eligible.

Premium Amount for Niramaya Health Insurance Scheme

In order to enroll or renew this Niramaya Scheme, beneficiary should pay an amount equals to the following:-

APL Category (Actual Premium)	Rs. 500
BPL Category (Actual Premium)	Rs. 250
APL Category (Renewal Premium)	Rs. 250
BPL Category (Renewal Premium)	Rs. 50

An affordable health insurance scheme for Persons with Disabilities covering under National Trust Act 1999. Through this scheme the beneficiaries can avail an insurance coverage up to 1 lakh, and it would be renewed every year. Facility for OPD treatment including the medicines, pathology, diagnostic tests etc., regular medical checkup for non – ailing disabled, Dental preventive dentistry, surgery to prevent further aggravation of disability, non-surgical/hospitalization, corrective surgeries for existing disability, ongoing therapies to reduce the impact of disability related complications , alternative medicine.
