“Unique ID for Persons with Disabilities” project is being implemented with a view of creating a National Database for PwDs, and to issue a Unique Disability Identity Card to each person with disabilities. The project will not only encourage transparency, efficiency, and ease of delivering the government benefits to the person with disabilities but also ensure uniformity.
The project will also help in streamlining the tracking of the physical and financial progress of beneficiary at all levels of a hierarchy of implementation – from village level, block level, District level, State level, and National level.

- If the child does not live with the parents, the name, address and phone number of the current residence

- Specify APL / BPL.
- Type and percentage of variance

- Copy of certificate proving disability
- Copy of Identity Document.
- Copy of Address Document (Ration Card, Aadhaar Card/Election ID Card/Ward Councillor’s Letter.
- Copy of certificates or documents proving competence in the field of arts, if any.
