A self-declaration form is used by individuals infected with COVID-19 to report their medical status. Self-declaration is used while given an exam at the exam center regarding a health issue. A self-declaration is a statement from the client, attesting to an expense amount or other circumstances of the case. There are three types of self-declarations: Verbal – a verbal declaration from the client with no signature. Self-declaration does not include any documentation other than the signature of the person making the statement.
Self-declarations are commonly used by litigants to submit factual evidence, such as witness statements or documentary evidence. Unlike affidavits, however, declarations are not formally sworn before a notary public or other officer authorized to administer oaths. Declaration Form means the form in which an exporter or importer declares the details of goods to be exported or imported.
