SBI Health Insurance Claim Form - Summary
SBI Health Insurance Claim Form
SBI Health Insurance provides an easy and efficient way to manage health insurance claims, thanks to its partnership with over 3000 hospitals across India. To make the claims process smooth and hassle-free, SBI Insurance has established dedicated health claim unit desks at all its tied-up hospitals.
Additionally, these hospitals have appointed Third Party Administrators (TPA), who are licensed by the IRDA, to help policyholders with their claims.
In the Situation of Treatment Expense Reimbursement
- If you are treated in a non-registered hospital, the insurance company might not approve your claims. In such cases, here are the steps that you can take:
- In an emergency, the policyholder’s family should contact the insurance company or TPA via phone or email. You can reach the toll-free helpline at 1800 22 9090.
- It is vital to pay the hospital bills immediately and keep all original bills and documents safe, as these will need to be submitted with your claim.
- Ensure you fill out the claim form completely and correctly, then submit it to RCare for reimbursement processing.
Documents Required for Claim
To get reimbursement for your medical expenses and treatments, you must provide original copies of the following documents:
- Details of the hospital
- Original hospital bills
- Discharge summary
- Other relevant medical reports
- Claim form
SBI Health Insurance reserves the right to deny claims for cashless hospital treatments under certain conditions. Nevertheless, in these instances, you can still apply for reimbursement by following the steps mentioned above.
