Submit the form: After carefully filling this form and attaching the necessary documents, submit the form to the LIC branch.
LIC NEFT Form – Documents required:-
- Cancelled cheque leaf with name and account number printed (or)
- Cancelled cheque leaf along with a photocopy of the page of Bank passbook containing the details of the account holder if name and account number not printed on the cheque leaf.
NEFT option required to the holders/claimants
- The NEFT application form should be sent to our branch address or customer zone.
- The policyholder/claimant will have to submit his unsigned check sheet or a photocopy of the passbook/checkbook containing complete details of his account.
- If you do not have a deposit in your bank account within two days of the due date, please contact your branch.
- The account of the policyholder or annuitant will be operated only at the time of payment of the policy.
