Ex Servicemen Employment Index Card Application Form (X-Card) PDF

The Ex Servicemen Employment Index Card Application Form (X-Card) helps veterans in India access essential benefits. Approved by the Government of India, the Ex-Servicemen Contributory Health Scheme (ECHS) began on April 1, 2003, providing vital healthcare support. This publicly funded medicare initiative is tailored for ex-servicemen and pensioners and their eligible dependents, granting them access to medical care. The scheme includes outpatient treatment at 227 polyclinics across India, in-patient hospitalization, and treatment at Military Hospitals, as well as at outsourced Civil Hospitals and Diagnostic Centers found at these facilities. ECHS members can also receive treatment in Service Hospitals when specialty medical staff and bed space are available.

Eligibility for ECHS

Joining the ECHS is open to all eligible veterans under simple criteria:

• Armed Forces Veterans (AFV) and their dependents who are receiving a pension or a disability pension.
• Widows and family pensioners who receive a regular or special family pension.
• Veterans of the Armed Forces (AFV) receiving a disability pension.

Joining the ECHS

For AFV or eligible individuals who retired on or before March 31, 2003, joining the ECHS is optional. When applying for membership or joining the scheme, there are no age or medical condition limitations. Importantly, there is no specific deadline for joining the ECHS. However, it’s crucial to note that ESMs who do not join ECHS will be ineligible for medical care at Service Hospitals after March 31, 2008.
