The Government of India is cognizant that primary Health Care is currently focusing largely on Reproductive, Maternal, Newborn, Child and Adolescent Health (RMNCH+A) and communicable diseases. This poses a challenge given the epidemiological transition, where non-communicable diseases account for nearly 62% of deaths among men and 52% among women – most of which are premature in nature.
हरियाणा सरकार ने आयुष्मान भारत योजना का शुभारंभ कर दिया है। यह योजना 21 नवंबर 2022 से शुरू कर दी गई है। इस योजना के अंतर्गत 28 लाख गरीब परिवारों को 5 लाख तक का स्वास्थ्य बीमा दिया जाएगा। हरियाणा आयुष्मान कार्ड की लिस्ट यहाँ से डाउनलोड की जा सकती है। इसमे सभी गाँव, ब्लॉक, जिले आदि की लिस्ट दी गई है। मुख्य मंत्री श्री मनोहर लाल खट्टर ने इस योजना का शुभारंभ करते हुए इसका नाम चिरायु योजना (Chirayu Scheme) रखा है।
Ayushman Card List Haryana 2024 – How to Download
Step 1:- In order to download the Ayushman Card List 2024 PDF visit the official website https://ayushmanbharat.haryana.gov.in
Step 2:- After visiting the official website you have to click on “Download Ayushman Card PMJAY” as the image is shown below
Step 3:- Then a new window is open in which you have to select the “Scheme”,”State” and then enter than Aadhaar Number / Virtual ID. After that, you have to generate the OTP and you can download the Ayushman Card List Haryana PDF as the image is shown below.
Ayushman Card List Haryana – Documents Required
- Aadhaar Card for Biometric Verification
- Family documents like Ration Cards/PM Letters/CM Letters
- Parivar Pehchan Patra (PPP ID) for family verification
- If a correction in SECC personal/family details is required by the beneficiary then a DNO Recommendation letter along with verification
letter from Sarpanch/Municipal Councilor is a must.
