Amarnath Yatra Medical Form 2026 - Summary
The Amarnath Yatra Medical Form PDF is the English Compulsory Health Certificate (CHC) for Shri Amarnathji Yatra 2026, issued in the format published by the Shri Amarnathji Shrine Board. The one-page form has an applicant section, medical history questions and a section for the authorised medical authority to certify fitness.
The Shrine Board says a valid 2026 CHC must be issued on or after 8 April 2026, with the applicant’s photograph attested by the authorised doctor. The certificate must use the pilgrim’s details and be completed by a doctor or institution on the Board’s authorised CHC provider list.
