NTA Self Declaration Form 2025 - Summary
Due to the ongoing coronavirus pandemic, students must now submit a self-declaration form along with a photograph. This form states that they have not tested positive for the novel coronavirus and have not shown any symptoms such as fever, cough, or breathing issues in recent days.
The self-declaration form is an important part of the exam process and must be presented at the examination center. Candidates should complete and sign this form in the presence of the invigilator before taking the exam. The self-declaration form for JEE Mains is included in the JEE Main admit card and can be found printed on the front page of the hall ticket. Every candidate is required to fill it out and sign it in order to be eligible to appear for the exam.

- Open the official website at jeemain.nta.nic.in.
- Click on the ‘download JEE Main admit card’ link.
- Login using the required credentials.
- Click on the ‘download admit card’ link.
- You will find the JEE Main self-declaration form printed on the first page of the admit card.
- Download and print all pages of the admit card for your records.
It is important to remember that no candidate will be allowed to enter the examination center without an admit card, valid ID proof, and proper frisking.
