1. It is our endeavour to settle your maturity claim on or before the due date. The servicing Branch usually sends maturity claim intimations two months in advance.
2. Please submit your Discharged Receipt in Form No.3825 with original policy document at least one month before the due date so that the payment is received before the due date of maturity claim.
3. If you have not received any intimation for your claim due in the next two months, please contact the servicing Branch immediately.

I am unable to take a print out due to the size which is difficult to avail in most of the printers. Need to re-size to suit in A4 size or A3 size with English , Hindi, Malayalam or other languages for sizing will help the customers..If you have an A4 Size of this form (3825)please sent me the earliest.
