Form 35 - Summary

Understanding Form 35
Form 35 must be filled out in duplicate, and in triplicate if the original registering authority is different. The duplicate and triplicate copies, along with the endorsement of the registering authority, must be returned to the financier and the registering authority at the same time as the termination entry is made in the registration certificate and Form 24.

- The appeal in Form 35 must be verified by someone who is authorized to check income tax returns.
- If a person qualifies to file their income tax return on paper, they can also submit an appeal in Form 35 either electronically using a digital signature or verification code, or in paper form.
