Steps to Change Name or Surname

For an adult applying for a name/surname change, it is necessary to attach a recently taken passport-sized photo signed on the application form. The old name should be replaced by the new name, and the complete address details must be mentioned in the affidavit. Along with the form, one photo ID such as an election identity card, PAN card, Aadhaar card, driving license, passport, ration card, telephone bill, light bill, or water bill must be submitted, along with one proof of residence. A total of two self-attested/certified copies of these documents are required.

The application for publication in the gazette must be submitted in original on non-judicial stamp paper, along with the affidavit for gazette publication, signed by the First-Class Magistrate, Executive Magistrate, or a Notary.
