Please visit the clinic or government hospital (where a registered medical practitioner who can issue such a certificate is available) OR search for your nearest doctor.
Medical Certificate Form PDF can be downloaded from the link given at the bottom of this page. Fill this form and attach a passport size photograph then medical practitioners check this form and after that if you need the applicant may have to take the tests in a lab.
Once the test result is issued or in some cases, if the test is not required, an applicant will be issued with a medical certificate.
