J&K Disabled Persons Permanent Identity Card (ID) Form
There are no caste and income criteria for handicapped members. All male handicapped who are are permanent residents of J&K state with a disability of 40% or more, unemployed.
1. To obtain an ID card for disabled, an applicant has to contact the respective “Superintendent/In-Charge of Medical Colleges & Hospitals and Civil Hospitals/CHCs/PHCs/SHCs/SDs.” for their area with the documents.
2. Please submit the completed application along with the required documents to the authority. After validation and checking of the document are done, the further process will be carried out.
3. The applicant has to follow the guidelines and instructions to complete the department process for considering the application to grant a card.
4. Finally, Disability Card will be provided by the respective “Department of Social Welfare”.
