EPF Return Form 11 (Self Declaration Form)
EPF form 11 is a self-declaration form that has to be filled and submitted by an employee at the time of joining a new organization that offers the EPF Scheme (Employees Provident Fund). Form 11 contains the EPF history of the employee and is mandatory for an employee to fill it upon joining an organization.

- Whether earlier a member of Employees’ Provident Fund Scheme, 1952
- Whether earlier a member of Employees’ Pension Scheme, 1995
- Previous employment details: [if Yes to 7 AND/OR 8 above]

- c) Date of exit from previous employment: (DD/MM/YYYY)
- d) Scheme Certificate No. (if issued)
- e) Pension Payment Order (PPO) No. (if issued)

- d) Validity of passport [(DD/MM/YYYY) to (DD/MM/YYYY)]
- KYC Details: (attach self attested copies of following KYCS)
