Bajaj Allianz Motor Insurance Claim Form - Summary

Bajaj Allianz General Insurance now trades as Bajaj General Insurance Limited, and the motor claim form published on its official site is the Bajaj General Motor Claim Form. This is the document a policyholder files after an accident or a theft, and it carries the list of papers the insurer expects along with the form.

The form asks for the policy number, the name of the insured, a copy of the registration certificate and a copy of the driving licence of the person who was driving the vehicle at the material time of the incident. It also asks whether the salvage of the damaged vehicle is to be retained, and takes the bank NEFT details of the policy holder for the claim payment, with a note that an original cancelled cheque carrying the name must be submitted if the claim is admissible.

One line on the form says that submitting documents or bank details does not, in any way, imply admission of the claim or liability by the company. Another asks for consent to upload KYC records on the Central KYC Registry portal under the IRDAI circular dated 12 July 2016, and to verify identity and address through CKYCR, GST, MCA, NSDL or UIDAI portals.

  • Copy of the registration certificate, which is mandatory
