Understanding the CGHS Application Form
The Central Government Health Scheme (CGHS) issues a plastic card known as the CGHS card to all beneficiaries. This card, equipped with a unique ID and a photo of the individual, is mandatory for availing CGHS services. It is important to note that the CGHS card cannot be transferred from one person to another.

- Retired judges of High Courts

CGHS Scheme Health Coverage
The Central Government Health Scheme offers comprehensive health coverage which includes:
- Out Patient Department (OPD) treatment along with necessary medicines.
- Reimbursement for expenses related to hearing aids, appliances, artificial limbs, etc.
- Coverage for treatment costs incurred in public or private hospitals during medical emergencies.
- Services related to Maternity, Family Welfare, and Child Health Care.
- Medical consultation expenses for all types of treatments including AYUSH (Ayurveda, Homeopathy, Unani, and Siddha).
Documents Required for CGHS Application Form
To apply for the CGHS card, the following documents are necessary:
- Proof of residence
- Age proof of the dependent
- Proof of stay of the dependent
- Attested copies of PPO/Provisional PPO/Last Pay Certificate
- Demand draft payable at “PAO CGHS New Delhi”
Provision for Making CGHS Card while Still in Service
Employees currently serving in a central government organization can apply for their pensioner CGHS card while still in service. They need to submit the application along with the required documents and the demand draft through their department. The pensioner card will only be issued upon retirement.
- If a serving employee gets married or has a child, they can add the spouse’s or child’s name by submitting the endorsed form from their department.
- If the cardholder passes away, the existing card will be invalid, and the dependents must apply for a new card with the old CGHS card and death certificate.
- If a dependent of the cardholder passes away, the CGHS card details can be updated by informing the CGHS department.
