Ayushman Card List UP 2025 - Summary
Ayushman Bharat Yojana List of beneficiaries is now available for easy download from the official portal pmjay.gov.in or mera.pmjay.gov.in. Now you can check your eligibility for Pradhan Mantri Jan Arogya Yojana or Ayushman Bharat Scheme. The PMJAY aims to provide Universal Health Coverage of Rs. 5 lakh per family per annum to around 10 crore families (50 crore people). Importantly, there are no restrictions based on caste, age, or income.
If you wish to enjoy cashless treatment of up to Rs. 5 lakh under the PM Jan Arogya Yojana, you can check your name in the Ayushman Bharat Yojana list UP 2022 of beneficiaries available for download at mera.pmjay.gov.in. The process to check your name in the PMJAY final beneficiary list is very simple. You can easily enter your registered mobile number, ration card number, SECC-2011 Name, or RSBY URN.
Ayushman Card List UP – How to Download
Step 1: Firstly, visit the official “Am I Eligible Portal” at mera.pmjay.gov.in.
Step 2: On the homepage, you need to enter an active mobile number to receive an OTP for finding your name in the Ayushman Bharat List UP as shown below:
Step 3: Click on the “Generate OTP” button to receive the OTP on your entered mobile number. After that, enter this OTP and click on the “Submit” button to go to the “Name Find Page”. Here, you will enter your details to find names in the Ayushman Bharat Yojana final beneficiaries list UP.
How to Download the Ayushman Bharat Card?
Step 1: Go to https://ayushmanup.in/. Scroll down to find the Quick Link that says “# Register Yourself on SETU”.
Step 2: Click the link, which will take you to the NHA’s Setu portal at https://setu.pmjay.gov.in/setu/.
Step 3: You will need to register by filling in all the required information and then clicking on Submit.

Step 4: After successful registration, you will need to complete your KYC and wait for approval. Once your card is ready and approved by the competent authority, you can download the card.
PM Jan Arogya Yojana Helpline Number (14555)
You can also call the PMJAY helpline number 14555 to check your eligibility. Ayushman Mitras are available at empanelled hospitals to assist patients and help coordinate with beneficiaries. These Arogya Mitras run a Help Desk and check documents to verify eligibility and enrollment in the scheme.
All beneficiaries have received letters with QR codes that will be scanned for demographic authentication to confirm their identification. The PMJAY Scheme aims to support poor and deprived rural families, along with specific occupational categories of urban worker families—covering 8.03 crore in rural and 2.33 crore in urban areas, based on the SECC-2011 database.
