Assam Ration Card Eligibility Criteria

  • Permanent Residency: The applicant and family members must be bonafide permanent residents of Assam.

  • No Existing Active Card: The applicant or listed family members must not hold another functional ration card in Assam or any other Indian state.

Documents Required for Assam Ration Card Application

1. For New Ration Card (Form C)

  • Complete particulars and passport-sized photographs of all family members (attested by Head of Family).
  • Copy of Aadhaar Card for all members (linked with active mobile numbers).
  • Birth certificates for minor children below 10 years of age.
  • Certified extract copy of the relevant page of the current Voter List (Electoral Roll).
  • Income Certificate issued by the Circle Officer / competent revenue authority (income < ₹4 Lakh).

  • Birth Certificate (for newly born children) or Marriage Certificate / Surrender Certificate from in-laws’ card (for newlywed brides).
  • Aadhaar Card copy of the member to be added.

3. For Member Deletion / Surrender

  • Original Ration Card.
  • Death Certificate (in case of demise of a family member).
  • Marriage Certificate or Migration proof (for deletion of married daughters).

4. For Duplicate Ration Card / Address Change

  • Self-declaration form explaining the reason for loss or damage.
  • Police Report / General Diary (GD) entry copy in case of lost or stolen card.
  • New residential address proof and NOC from the previous Fair Price Shop (for shop/address transfer).

How to Apply for Assam Ration Card

Offline Application Method

  1. Download the official Assam Ration Card Form C (PDF) from the link provided below or obtain a physical form from your local Gaon Panchayat office, Urban Local Body, or Circle Office.
  2. Fill in the head of family details, complete address, and details of all dependent family members.
  3. Attach all required supporting documents (Aadhaar, income certificate, residential proof, voter list extract).

How to Complete Assam Ration Card e-KYC

To ensure uninterrupted free ration supplies, follow these simple steps to complete your mandatory e-KYC:

  1. Visit your designated Fair Price Shop (FPS) ration dealer with your original Ration Card.
  2. Each family member whose name appears on the card must present their original Aadhaar Card.
  3. The dealer will input your ration card number into the electronic Point of Sale (ePoS) device.
  4. Each member must place their thumb or finger on the biometric scanner for Aadhaar authentication.
  5. Once the fingerprint matches UIDAI records, the e-KYC status will update to “Verified” on the RCMS database.
