The RTO Form 1A is the second application in the category of “Driving Licence.” When applying for a driver’s license, applicants must provide this document along with Form 1.
Before filling out this application, the state government chooses a qualified medical practitioner to conduct a complete medical assessment on candidates. This document also allows the doctor to double-check the information provided on Form 1.
Documents Required

Important Info
- The medical practitioner’s signature should be affixed to the candidate’s passport-size photograph in such a way that part of it is over the photo and the rest on the application.
- An applicant who is deaf but not deaf is also qualified to apply for non-transport vehicles.
- Attach a recent photograph with a dark background to your face so that your features are clearly seen.
