AP Meeseva FP Shop Authorization/Renewal Form
FP (Fair Shop) Shop Renewal request is applied by the citizen through the Meeseva center. This request can
be accessed by the RDO and processed. List of Documents are to be enclosed with this form:-
1. Copy of Authorization letter
2. Latest renewed food grains and kerosene License
3. PHC Certificate (Certificate is enclosed in case he is physically handicapped)
