Aarogyasri Health Card In Andhra and Telangana sectors for the people who are below poverty line and any other peoples whose name and photo appear on BPL ration card or Health card and suffering from identified diseases are eligible to avail benefits under this scheme. The annual income of the family should be below Rs. 5 lakhs while applying for the scheme.
List Of Documents For Aarogyasri Scheme:-

• Passport Size photo.
• Attach the Aadhaar card number ( if you apply for new Aadhaar card you can enter the enrollment number as well )
• Disabled certificate ( attach the disability certificate if you have )
• Date of birth certificate.
