AP Meeseva Birth Certificate Application Form
Application for the Birth certificate can be collected from the Meeseva center. Fill the application form and submit the form along with documents and fees to the Meeseva officer at the counter.
The details will be processed online. Applicant number will be given to you for future reference
The birth certificate will be issued if the details match with hospital records.
Required Documents:-
- If the birth takes place at a dispensary/hospital, the register record is enough.
- Address proof(Voter ID, Driving License, etc.) of parents
- Aadhaar Card of Parents
- If the birth takes place at a house, the citizen has to approach the local body and inform in writing. Then it will be verified & the certificate will be issued.
- The application is to be made in the prescribed format of Form-2.
