LIC Maturity Form
LIC Maturity Form PDF is the Application form issued by the LIC of India and this form can be used by the policyholders to submit this form once your policy matures. This form can be downloaded from the official website of LIC of India i.e https://licindia.in or it can be directly downloaded in PDF format from the link given at the bottom of this page.
Please submit your Discharged Receipt in Form No.3825 with the original policy document at least one month before the due date so that the payment is received before the due date of the maturity claim.
LIC Maturity Form – Claims
- It is our endeavor to settle your maturity claim on or before the due date. The servicing Branch usually sends maturity claim intimations two months in advance.
- Please submit your Discharged Receipt in Form No.3825 with the original policy document at least one month before the due date so that the payment is received before the due date of the maturity claim.
- If you have not received any intimation for your claim due in the next two months, please contact the servicing Branch immediately.
