Applicants must include their basic details, permanent address, Aadhaar number, Khadyasathi number for the family, Krishak Bandhu number, Fisherman Card number, Artisan Card number, MGNREGS (100 days’ work) Job Card number, bank account details, relief assistance being applied for, and any other relevant information.
Submit Form: After filling out the form carefully, you must submit it at the district level to the District Task Force. For urban areas, the Sub-Division Level Task Force will be formed by the SDOs, and at the block level, the Block Level Task Force will handle the submissions.

The initiative “Duare Tran” is set to operate in the affected districts of South 24 Parganas, North 24 Parganas, Purba Medinipur, Paschim Medinipur, and Howrah. It will also cover areas impacted by tornadoes in North 24 Parganas, Birbhum, and Hoogly.
Through Duare Tran, outreach programmes and camps will be organized in the affected areas at the levels of Gram Panchayat and Municipal Ward. This will empower eligible citizens to submit their applications on plain paper and receive benefits, relief, or compensation from the Departments of Agriculture, Disaster Management and Civil Defence, Animal Resources Development, Horticulture, Fisheries, and Micro, Small and Medium Enterprises and Textiles.
