This is an application form for Current and saving account opening form issued by the Axis Bank and this form can be obtained from the nearest branch of Axis bank or it can be directly download from the link given below in PDF format.

An individual residing in India who does not have existing savings account in Axis Bank, prior to opening savings Account (If an existing account exists, it needs to be closed in order for the individual to be eligible for the Basic Savings Account and once the Basic Savings Account is opened, the individual cannot own another savings account in Axis Bank)

Documents for Savings Account for an Individual:

• Two latest passport size photographs
• Address and identity proof documents
• Identity proof documents:-
• Proof of Possession of Aadhaar number
• Passport
• Driving Licence
• Voters ID Card (Issued by Election Commission of India)
• Job Card by NREGA duly signed by an officer of the State Government
• Only a single copy of the document is required if it is listed under both address and identity proof, e.g. Passport / Aadhar Card
• If your mailing address and permanent address are different, you must provide address proof documents for both

Documents for Savings Account for a Hindu Undivided Family

• Copy of Aadhaar Card / Form 60 of HUF
• Declaration from the Karta
• Proof of identification and address of Karta as per documentation for individual
• Prescribed Joint Hindu Family Letter signed by all the adult holders

Documents for Joint Basic Savings Account

• Address and identity proof documents for both applicants
• Address proof can be only of the first holder of the account, as long as it is supplemented by a relationship proof between both holders
